• How do I see my claims online?

    It’s easy! You can view your claims through the Pivot Health mobile app or by registering with the claims administrator online at its self-service login. With single sign-on technology, you can view your claims history in just a few clicks.

  • How do I get a doctor referral?

    You do not need a referral to see a specific doctor. However, most procedures require pre-authorization in advance of an appointment. Contact the pre-certification line on the back of your ID card to pre-certify: 866.317.5273. Failure to pre-certify carriers a reduction in benefits by $500.

  • What approved medications are covered?

    We recommend contacting the pharmacy telephone number on the back of your ID card to find out what medications are covered under the pharmacy formulary schedule. It changes yearly so drug coverage is subject to change.

  • How do I terminate my coverage?

    To end your plan policy, email Client Services at  clientservices@insurancebenefitadministrators.com with your plan ID number and the date you wish your coverage to terminate.
